Surrendering a Dog

We know there are a lot of reasons you may need to give up your puppy and we know it’s not always an easy decision.  Our goal is to find a loving home for every healthy and safe animal, and to know if our rescue is a fit, we ask you to fill out the following Surrender Form. This form will give us the basic information we need to know for your puppy.

• Please note: while we do consider adult dogs at times, our focus is to rehome or place puppies that are six months or younger.
• If you have an adult you are looking to rehome, we still welcome you to fill out our Surrender Form as if we are not able to help, we may be able to refer you to another rescue that can.
• We are located near Rochester, MN and may ask for help transporting your puppy into our rescue.

If you have a Forever Fido dog that you need to return, please email us at info@foreverfido.org.  Per the adoption contract, the dog/puppy must be returned to Forever Fido.
